What Pennsylvania requires

Notice / filing	Required — see the filings below Notarized Affidavit — Submit notarized affidavit and objectives to superintendent. Annual Evaluation — Evaluator's written certification of an appropriate education, due June 30. In grades 3, 5 and 8 the portfolio your evaluator reviews must also include results of a nationally normed standardized test (or the statewide test) in reading and math — someone other than you gives the test.

Moving to or from Pennsylvania
